Implication of thoracic aortic stent-graft conformability on clinical outcomes.

چکیده
Endovascular management concepts have recently emerged as valuable treatment options for both simple and complex disorders involving both the distal arch and descending thoracic aorta. With growing use of endovascular stent-grafts the technique has generated broad interest. Nevertheless, an ideal device for different thoracic aortic conditions is not available yet. After initially using the first-generation or "home-made" thoracic stent-grafts with its relative rigidity and considerable diameter of the delivery system, recent advances have generated more flexible and conformable second- and third-generation stent-grafts. Nonetheless, challenges of providing optimal conformability and better fixation in the aortic arch and thoracic aorta are substantial, given the spiralling motion of the thoracic aorta with each ventricular contraction. Ultimately, the capacity of thoracic stent-graft to appose tightly to the aortic wall not only ensures effective sealing and fixation, but also avoids endoleaks and migration, graft stability and thus overall successful repair with aortic remodeling. Conformability is therefore the combined effect of apposition to the thoracic aortic anatomy, and kink-resistant flexibility of the stent-graft.

Contemporary Role of Computational Analysis in Endovascular Treatment for Thoracic Aortic Disease.
In the past decade, thoracic endovascular aortic repair (TEVAR) has become the primary treatment option in descending aneurysm and dissection. The clinical outcome of this minimally invasive technique is strictly related to an appropriate patient/stent graft selection, hemodynamic interactions, and operator skills. In this context, a quantitative assessment of the biomechanical stress induced i...
متن کاملEndovascular Repair in Acute Complicated Type B Aortic Dissection: 3-Year Results from the Valiant US Investigational Device Exemption Study
Acute complicated type B aortic dissection (TBAD) is a potentially catastrophic, life-threatening condition. If left untreated, there is a high risk of aortic rupture, irreversible organ or limb damage, or death. Several risk factors have been associated with acute complicated TBAD, including age and refractory hypertension. In the acute phase, even uncomplicated patients are more prone to deve...
متن کاملThe Importance of Conformability and Sizing in TEVAR
T he management of patients who suffer blunt aortic injury (BAI) has drastically changed since the approval of stent grafts to treat aortic disease. At most major trauma centers, thoracic endovascular aortic repair (TEVAR) has now become the treatment of choice.1 With the recent approval of the Conformable GORE® TAG® Device (Gore & Associates, Flagstaff, AZ) for isolated lesions, BAI is now an ...

BACKGROUND The aim of this study was to analyze device conformability in TEVAR of acute and chronic (a/c) type B aortic dissections (TBAD) using the Gore Conformable Thoracic Aortic Stent-graft (CTAG). MATERIAL AND METHODS From January 1997 to February 2014, a total of 90 out of 405 patients in our center received TEVAR for TBAD. Since November 2009, 23 patients (16 men; median age: 62 years) w...
متن کاملImportance of dynamic aortic evaluation in planning TEVAR.
Dynamic aortic evaluation in planning thoracic endovascular aortic repair (TEVAR) is important to provide optimal stent graft sizing. Static imaging protocols do not consider normal aortic dynamics and may lead to stent graft to aorta mismatch, causing stent graft related complications, such as type I endoleak and stent graft migration. Dynamic imaging can assist in accurate stent graft selecti...
